Insufficient evidence returns a refusal.
If the selected payer, period, or axis lacks enough government-filing evidence, Telamon returns INSUFFICIENT_EVIDENCE with the shortfall itemized.
The response never substitutes a benchmark estimate for a missing filing-backed value. This keeps negotiation material tied to documents that can be cited.
